Walked into 2026 with $4.3M of annual UM operating spend and CMS-0057-F public-reporting deadlines they could not credibly hit on the legacy stack. Authora pilot ran 12,400 cases across MSK surgical, imaging, oncology drug, behavioral health, and DME from May–July 2026. P50 turnaround time dropped from 11.2 days to 26 seconds. Six of the seven pilot categories now run unattended for the auto-approve track.

The system's CMO ran the math: 460 physicians × 13 hours per week on PA × $280/hour internal loaded cost = $87M of physician time spent on PA per year. Authora deployed inside Epic via SMART-on-FHIR + CDS Hooks, with the PA workflow living in the EHR. Physicians sign the order, Authora pre-flights, the request goes out. Peer-to-peer load dropped 72% because gaps surface at order time, not after a denial.

Took delegated UM from two regional Medicare Advantage plans in 2024. By Q3 2025 the manual UM operation cost them $1.74M/year and was a recurring NCQA audit-finding source. Authora pilot replaced the manual workflow with a delegated agentic pipeline + parallel reviewer console; the operation now runs at $4.40 per approved auth, fully loaded, with NCQA UM audit files generated automatically every quarter.


The common thread

What the pilots have in common.

Across all three pilots, the same pattern: the customer’s appeals overturn rate — the share of denied PAs that get reversed on appeal — drops sharply within the first 30 days. The reason is structural. Authora surfaces documentation gaps at order time, before the request leaves the building. Cases that would have been denied for missing evidence get the missing evidence attached first. The denials that do go out are the ones that actually deserve to. That is the only durable answer to the KFF data on overturn rates.
